Welcome to Revital Health’s new online repeat prescription service!
We’re changing the way repeats of your product prescriptions from Revital Health can be ordered – from October 23rd, 2024 please use the form below to order any repeat prescriptions of your products (including herbals, compounds, and supplements).
Once we have received your completed form, our team will put together an invoice and send this to the email address we have on file for you.
After payment has been made, we will have your products prepared and ready for you at the earliest convenience.
If you are collecting your products from our clinic: An SMS will be sent to notify you when your products are available for collection.
If your products are being posted to you: An email with your parcel’s tracking number will be sent to you when your products have been shipped.
*If your last appointment was over 3 months ago, we are unable to issue you with a repeat prescription. This is to ensure that we are maintaining the highest standards of practice for your ongoing treatment at Revital Health.
